Through MyChart, you can schedule appointments, view test results, review medical bills, request prescription renewals, and access a wealth of information about your medical care—all in one convenient place.
We need some information in order to set up a MyChart account account. Use the link below to enter your demographics which we will use to verify your identity using questions from a third-party verification system. Once verified, you will be asked to set up a username and password.

Parents and caregivers can access and manage their loved one’s personal and family health information online by submitting a MyChart proxy access form. Proxy access allows an authorized individual—such as a family caregiver, home health aide, parent or guardian, adult child, or healthcare power of attorney—to view a patient’s portal.

If you are 18 years of age or older and would like another adult to access your medical information contained in MyChart, then you, as the patient, should complete this form. If you are 13-17 years old, you may only request proxy access for a parent or legal guardian, and you should complete the form below.

No visitors are permitted in the hospital from 8:30 p.m. – 7 a.m., except for those accompanying maternity or pediatric patients.
The state of Ohio is experiencing widespread influenza. At Mercer Health, we are seeing influenza patients in our outpatient areas, as well as patients that have been admitted for care and treatment related to influenza. As a precaution, we are limiting visitors at Mercer County Community Hospital effective February 5, 2025.

Patrons of The Cedar Table are asked to park in the West/#3 Parking Lot and enter at the West Entrance/#3 or Entrance #4 (located off of the Northwest Pavilion). See Mercer Health’s campus map for details.

Visit the Gift ShopPastoral Care and Chaplain Services are available to serve the spiritual needs of patients, families and staff. Our chaplains are available to listen to concerns, provide prayer and offer spiritual support.

The Chapel at Mercer Health is located near the East entrance of the hospital. This beautiful and peaceful space is newly renovated and available 24 hours a day, 7 days a week, for use by patients and their families.
