Mercer Health Home Care offers skilled services to patients in their place of residence. We also can provide home medical equipment for patients at home. We serve clients in Mercer County, Ohio, and the fringe areas of Van Wert, Shelby, Auglaize and Darke counties.

Mercer Health Medical Equipment offers both durable medical equipment (DME) and routine home medical equipment (HME) to assist you in your home. This ranges from ambulation aids to oxygen/CPAP equipment. We have Respiratory Therapists and Customer Services representatives on staff to provide high quality, professional set-up and teaching related to respiratory needs and equipment.

Mercer Health Home Care provides skilled services under a provider-approved plan of care for the purpose of promoting, maintaining, and restoring health or minimizing the effects of illness or disability. Our staff includes registered nurses, certified wound/ostomy nurses, aides, therapists, dieticians, social workers and respiratory therapists and we offer care for all ages from newborns to the elderly.

Perform clinical assessments to determine home care needs and assist with skilled procedures such as wound care, IV care, chronic disease management and medication management

Certified Ostomy/Wound Nurse:

Assist clients with all care related to ostomies and wounds

Home Care Aides:

Assist with bathing, dressing and other daily routines

Physical, Occupational & Speech Therapists:

Help clients gain strength and assist them in their recovery after surgery or other health problems

Registered Dietitians:

Help with nutrition counseling

At-Home Skilled Nursing Services

We provide skilled nursing assessment, nursing treatments, and comprehensive client and family education. The focus of the integrated programs is to treat clients to ensure optimal clinical and quality outcomes. Additional benefits include:

Skilled Therapy Services

Skilled therapies, including physical, occupational and speech can be provided in the home with physician orders helping to identify ways to safely remain in the home and optimizing your ability to care for yourself.

Telemonitoring Services

We offer a free wireless telemonitor system to home care patients which provides daily contact with a registered nurse who assesses the patient’s status and collaborates with the physicians as needed – no need to wait until the next home nurse visit.

Telemonitoring improves communication, allows us to more efficiently respond to health conditions and assists with the management of chronic conditions, which can result in increased quality of life and decreased hospitalization.

Home Medical Equipment

Mercer Health Medical Equipment provides a variety of home medical equipment, from accessibility aids to oxygen equipment, to assist patients in their homes. Our team includes respiratory therapists and customer service representatives who assist patients and families with respiratory equipment setup and education.

On-call and after office hours available 24/7.

We provide services 24 hours a day, 7 days a week and have skilled staff on-call after office hours for any problems or questions that occur. Please call the hospital at 419-678-2341 and request to talk to the Home Care or Medical Equipment staff member on call. They will take your name and number and contact the appropriate staff. The staff member will then call to assist you!
