MyChart is a free and secure online portal that gives you quick, convenient access to your health information.

Please do not use MyChart to send messages that require urgent attention.

Get answers to your medical questions from the comfort of your own home.

How do I message my provider?
Send a message to your provider by clicking the “Messages” button. Select “Send a message” and choose “Ask a medical question.” Then select message recipient (your provider) and message subject. Type your message, attach images if applicable and click “Send.”
Access your Test Results

No more waiting for a phone call or letter – view your results and your provider’s comments within days.

How do I review my test results?
Test results are available by clicking the “Test Results” button. Results will appear in a list that you can sort by name, ordering provider or date. Please note it is possible that you will view your results before your provider has had a chance to review them. Your provider will receive and review all your test results, and he or she will contact you if further discussion about your results are needed. If you have questions or concerns about your test results, please contact your healthcare provider.
Manage Medications & Request Prescription Refills

Send a refill request for any of your refillable medications.

How do I refill my prescriptions?
Refills can be requested via your MyChart account by clicking on the “Medications” button. Select your medication needing a refill and select “Request refill” option. Review your medication list to determine if any of your other medications are in need of refill and check next to the additional medication(s) then select “Next.” Select your delivery method and your preferred pharmacy and select “Next” and review the requested information and pharmacy information again. If all is correct, select “Submit.” If changes are needed, select the “Back” button and make your desired changes before submitting.
Manage your Appointments

Schedule your next appointment, or view details of your past and upcoming appointments.

How do I schedule an appointment?
Select “Schedule an Appointment.” Choose your provider, the reason for your visit and the day/time that works best for you. Please note—if you are a new patient or haven’t been seen by your provider in the past three years, online scheduling is not available. Please call your provider’s office to schedule an appointment.
View your notes after your visit

Forget to write down something you discussed with your provider? Easily access your After Visit Summary or appointment notes to review what was discussed during your appointment.

How do I view my visit notes?
Select “Appointments” then scroll down to “Past.” Here you can select “View After Visit Summary” or “View notes” under the appropriate office visit.
Find Pricing Estimates and Review Billing Statements

Although you currently cannot pay your bill through Mercer Health’s MyChart, you can review your billing statement and access pricing estimates. Bills can easily be paid online through the Mercer Health website.

What is MyChart?

What do I need to sign up for MyChart?

You need a computer, smartphone or tablet with access to the internet and a valid email address.

How do I sign up for MyChart?

Sign up at a Mercer Health or Mercer Health Medical Group location or online at anytime.

We need some information in order to set up a MyChart account account. Use the link below to enter your demographics which we will use to verify your identity using questions from a third-party verification system. Once verified, you will be asked to set up a username and password.

You can use MyChart on the web or on your phone or tablet using the MyChart app. On your mobile device, download MyChart from the Apple App Store or Google Play.

Is MyChart secure?

Yes. Your healthcare privacy is very important to us. Access to information is controlled through secure access codes, personal IDs and passwords. Each person controls their password, and the account can’t be accessed without that password. All MyChart messaging is done while you are securely logged in. For additional security, users can enable two-factor authentication.

How do I access another person's health records?

Parents and caregivers can access and manage their loved one’s personal and family health information online by submitting a MyChart proxy access form. Proxy access to the patient portal means that an authorized individual, such as a family caregiver, a home health aide, a parent or guardian, an adult child, or a healthcare power of attorney, may view an individual’s patient portal

If you are 18 years of age or older and would like another adult to access your medical information contained in MyChart, then you, as the patient, should complete this form. If you are 13-17 years old, you may only request proxy access for a parent or legal guardian, and you should complete the form below.

Use MyChart to schedule an Urgent Care Visit

Log on to your MyChart account and click “Menu.” Under “Find Care” select the “Urgent Care” option. If your location services are enabled, you can view the current wait time at the Mercer Health Emergency Department or “Reserve a time” at Mercer Health Urgent Care at Celina.

Easily Manage Your Care Online

Update your personal information:

Log on to your MyChart account and click “Menu”. Enter “Personal” in the search bar and click the “Personal Information” option. From here you can update your contact information such as home address, email, phone number, etc.
