What is MyChart?

MyChart is a free and secure online portal that provides information about your medical care and connects you to your health care team at Mercer Health. Schedule appointments, view test results, view medical bills, request prescription renewals and more.

How do I sign up for MyChart?

We need some information in order to set up a MyChart account account. Use the link below to enter your demographics which we will use to verify your identity using questions from a third-party verification system. Once verified, you will be asked to set up a username and password.

How do I access another person's health records?

Parents and caregivers can access and manage their loved one’s personal and family health information online by submitting a MyChart proxy access form. Proxy access to the patient portal means that an authorized individual, such as a family caregiver, a home health aide, a parent or guardian, an adult child, or a healthcare power of attorney, may view an individual’s patient portal

The online proxy form for parents of an unemancipated minor patient or legal guardian of a patient under the age of 13 can be found here (you will be prompted to login):

If you are 18 years of age or older and would like another adult to access your medical information contained in MyChart, then you, as the patient, should complete this form. If you are 13-17 years old, you may only request proxy access for a parent or legal guardian, and you should complete the form below.

Visiting Hours

Daily from 7:00 a.m. – 8:30 p.m.

No visitors are allowed in the hospital from 8:30 p.m. – 7 a.m., unless accompanying a maternity or pediatric patient.

Please note our visiting hours may change periodically to best protect our patients, staff and community.

Food & Dining Options

At Mercer Health, we deeply care about your comfort no matter how long your visit may be. We invite you to explore our onsite dining options.


The Cedar Table
The Cedar Table offers a full menu for breakfast and lunch, with grab and go options for dinner. A self-serve salad bar is also available during lunch. Menu items change daily.

The monthly menu can be found here or you can hear the menu each day by calling 419-678-4843, extension 5500.

The daily hours at The Cedar Table are:

  • ​Breakfast: 6:30 – 10:30 a.m.
  • Lunch: 11 a.m. – 1:30 p.m.
  • Closed: 2:00 – 3:00 p.m.
  • Dinner: 4:30 – 6:00 p.m.

Patrons of The Cedar Table are asked to park in the West/#3 Parking Lot and enter at the West Entrance/#3 or Entrance #4 (located off of the Northwest Pavilion). See Mercer Health’s campus map for details.

Vending Options
During those times when our serving area is closed, snacks and beverages may be purchased from the vending machines located near the East Entrance and in The Cedar Table dining area.

The Gift Shop at Mercer Health

If you would like to give a gift to someone you are visiting you can conveniently shop at our gift shop located on the first floor of the hospital.

Pastoral Care & Spiritual Services

Pastoral Care and Chaplain Services are available to serve the spiritual needs of patients, families and staff. Our chaplains are available to listen to concerns, provide prayer and offer spiritual support.

If you or your loved one would like to talk with a chaplain, contact a nursing supervisor or call 419-678-2341.

The Chapel at Mercer Health

The Chapel at Mercer Health is located near the East entrance of the hospital. This beautiful and peaceful space is newly renovated and available 24 hours a day, 7 days a week, for use by patients and their families.

Vendor Registration Policy

All vendor representatives visiting Mercer County Community Hospital will be required to sign in and obtain a badge at one of these convenient locations:

  • Materials Management Office
    (7:30 a.m. – 4:30 p.m.)
  • West Registration Desk
  • Emergency Registration Desk

At the end of your appointment, stop by any of these locations to sign out and return your badge.
