  • Ohioans ages 16 and older are eligible for vaccination.  (NOTE: With parental or legal guardian consent, individuals ages 16 and 17 are eligible to recieve the Pfizer vaccine, which is the only COVID-19 vaccine currently authorized for use with people under age 18.)  For more information, see the Ohio Department of Health's Vaccination Resources.
  • To Register  
    • Vaccine Registration can now be done via Mercer Health MyChart.
      • Select Appointments, then Schedule an appointment  and select the COVID Vaccination – Mercer Health option.
      • NOTE: If you choose to schedule your appointment via MyChart, you only need to schedule your first dose. We will schedule your second dose for you. 
        • TO SIGN UP FOR MYCHARTMyChart is a health management portal connecting you to your providers and to important information from your electronic medical record. With MyChart, you can not only schedule your COVID-19 vaccine but you can schedule other appointments, review test results, manage medications and request refills, send messages directly to your healthcare provider, view after visit summaries and access price estimates and review billing statements.
          1. To create your MyChart account, go to mychart.osu.edu/mercerhealth
          2. Click on Create Your Account
          3. On the right side of the screen, under “No Activation Code?” click “Create Your Account Online.” Enter your information and follow the registration prompts. 
    • Phone Registration is also available each week day (Monday - Friday), 9 a.m. - 5 p.m., by calling 567-890-7188.
  • CANCELLATIONS: Please be considerate if you have to cancel your appointment and provide notice at least 24 hours in advance. The vaccines arrive and must be dosed in groups and appointment no-shows jeopardize vaccine administration to all others scheduled at the same time. If you must cancel, you must call Mercer Health’s COVID-19 Hotline at 419-678-5395 for further direction.
